Skilled home health care assists a person in their recovery from an illness, accident, surgery, or change in their medical condition.
We offer nursing, rehabilitation therapy, home health aide and social work care to patients in the comfort of their home. When we receive a referral from your physician or nurse practitioner, our staff will work with them to determine exactly what care you need to recover. Together
with you, your family and your physician or nurse practitioner; we set goals that will speed up your recovery. As we work to help you get better,
we will stay in contact with your physician or nurse practitioner and keep them informed about your progress. We will also teach you and your family any skills you need to help you recover faster. We can also assist you with referrals to other community resources when needed.
